Instructions for filling out a chain-of-custody record

Project No. :
Your project number for this sampling event. The Project No. can be a file number, purchase order number, etc.
 
Project Name :
Your project description for this sampling event.
 
Sampled By (Signature) :
Please sign your name.
 
Sampled By (Printed) :
Please print your name.
 
Date :
The date the sample was taken.
 
Time :
The time the sample was taken.
 
Composite/Grab :
If several samples were composited to create this sample, place a “C” in this box. If the sample is a grab sample and not composited, place a “G” in this box.
 
Field Filtered :
If sample is field filtered, place a “Y” in this box, if not field filtered, then place an “N” in this box.
 
Sample ID No. :
Provide your sample ID number. This number or description will appear on the analytical report. (e.g., MW-2) Use one line per sampling location.
 
No. of Containers :
Number of bottles collected for the sample(s).
 
Parameters :
Write in the name of the tests to be performed. (e.g., Nitrate, BOD, COD, pH, etc.).
 
Remarks :
Available space for comments include field preparation, turnaround times or other pertinent details.
 
Relinquished By :
The signature of the person releasing custody of the sample(s). Sign your name, print your name, and enter the date and time.
 
Received at
Laboratory By :
To be completed by Continental Sample Receiving personnel.
 
Shipped Via :
How or what shipping courier is being used? (e.g., UPS, Fed X, Airborne, etc.)
 
Seal Number :
Enter a unique identifying number that corresponds to the Chain-of-Custody seal, which is placed on the cooler.
 
Send Report To :
The complete address and the person’s name the analytical report should be mailed to. Please provide a telephone number and fax number.
 
Send Invoice To :
The complete address and the person’s name the invoice should be mailed to. Please provide a telephone number and fax number.
 
Retain the yellow copy for your records. Seal the white copy in the plastic bag provided, place it in the cooler with the samples (preferably taped to the inside of the cooler lid), seal the cooler and ship it to the laboratory.
